Who We Are

The Greater Rochester Health Home Network, LLC. (GRHHN) was formed in January 2012 to establish a Health Home in order to serve the most vulnerable people in our community, especially those struggling with multiple medical and behavioral health conditions.

GRHHN was developed through a collaborative process with community providers to learn new ways to integrate with care team partners across medical, mental health, substance abuse, and community-based service sectors. Our goals are to:

  • Decrease Emergency Room and inpatient use.
  • Support access to health care services.
  • Link with community resources to provide comprehensive care.

In 2018 Finger Lakes Performing Provider System (FLPPS) acquired GRHHN. FLPPS is a partnership comprised of 19 hospitals, 6,700 healthcare providers and more than 600 healthcare and community-based organizations in a 13-county region (Allegany, Cayuga, Chemung, Genesee, Livingston, Monroe, Ontario, Orleans, Seneca, Steuben, Wayne, Wyoming and Yates counties).

This partnership works together to implement innovative system transformation using the NYS Delivery System Reform Incentive Payment (DSRIP) program to reform local Medicaid healthcare delivery. 

Meet Our Leadership Team

Peter Bauman LCSW-R, PMP is the Director of Health Home for GRHHN. Mr. Bauman has a 20  year history in the health care field. Recently he was the Director of Operations for the Finger Lakes PPS where he provided oversight for DSRIP projects and initiatives that aimed to fundamentally restructure healthcare delivery by  increasing PCP engagement and promoting interventions that address Social Determinants of health. Prior to FLPPS he held various leadership positions at managed care companies. He utilized his expertise to implement workflows and processes to improve patient quality by increasing patient and provider engagement. Peter is a Licensed Clinical Social worker,  who earned his Project Management Certification 2010 and is Certified Green Belt -Lean Six Sigma . His favorite role is that of husband and dad to four beautiful children who have made him a better human being.

Director of Health Home Operations

Peter Bauman LCSW-R, PMP is the Director of Health Home for GRHHN. Mr. Bauman has a 20+ year history in the health care field. Elizabeth Ortiz Jones, LPN, PMP is responsible for oversight of quality care at GRHHN, which includes auditing client records, processing incident reports and maintaining regular contact with Care Management Agencies. Her staff review plans of care and review audit results in compliance with NYS Health Home and NYS Department of Health standards. Liz brings more than 23 years of experience in the healthcare industry, including 15 years on the clinical floor as an LPN. Prior to joining GRHHN, Liz worked at MVP Health Care where her role as a Lead Population Health Management Professional supported our local independent physician association’s in value-based care arrangements.

Liz Jones
Quality Assurance Manager

Elizabeth Ortiz Jones, LPN, PMP is responsible for oversight of quality care at GRHHN, which includes auditing client records, processing incident reports and maintaining regular contact with Care Management Agencies. Her staff review plans of care and review audit results in compliance with NYS Health Home and NYS Department of Health standards. Liz brings more than 23 years of experience in the healthcare industry, including 15 years on the clinical floor as an LPN. David Mancuso
Marketing & Business Development Specialist

David develops and implements marketing plans, including direct marketing at provider offices to increase enrollments and educate referral agencies of the benefits of working with the GRHHN. David has more than 25 years of experience in the health care field, having previously worked at two large health care systems, and insurance provider and social services agencies helping those most in need in our community.
Amy oversees GRHHN’s Intake Department, which serves as the “front door” of the Health Home. The Intake Department answers questions regarding Health Home Care Management eligibility, facilitates communication between Care Management Agencies and community providers for current clients, and processes new referrals for clients looking to connect with Care Management services. Amy is a Licensed Master Social Worker and has more than 27 years of experience in the health care field, having previously worked as an Inpatient Social Worker in a one of the area’s largest hospitals and as both frontline and managerial staff in a local residential mental health rehabilitation program.

Amy oversees GRHHN’s Intake Department, which serves as the “front door” of the Health Home. The Intake Department answers questions regarding Health Home Care Management eligibility, facilitates communication between Care Management Agencies and community providers for current clients, and processes new referrals for clients looking to connect with Care Management services.
