As a lead health home, GRHHN is committed to providing our providers and care managers with the tools and training needed to provide the highest level of care to our patients. One of the roles of a lead Health Home is to ensure quality oversight across all partnering Care Management Agencies, including universal training for non-centralized care managers supporting patients.
